Appointments

What should I bring to my first appointment?

Bring a copy of your driver’s license and insurance card, list of medications and doses.

Do I need to arrive early to my appointment?

Please arrive about 15 minutes early to allow for check-in.

What should I expect after my appointment is completed?

At the completion of your appointment, your provider will make recommendations for other specialists, order prescriptions via our electronic system that go directly to the pharmacy of your choice if necessary and provide you with a detailed summary of your visit and future orders or referrals as well as education about your diagnosis.

Will I be charged a fee if I miss my appointment?

There is no fee charged for a missed appointment. However, the time has been reserved for you.

How much will I owe at the time of service?

Fees will vary based on your insurance plan.

What are my rights as a patient?

A detailed description of your rights is available at the time of your visit.

Where can I get a copy of my medical records and test results?

A copy of your medical records and test results is available to you via MyChart (the patient portal) or can be requested at the clinic.

How long will it take to get a referral?

Referrals time will vary depending on specialty and urgency of condition.

Billing and Payments

Who do I call about billing questions?

What administrative fees are there for forms completion (which is not covered by insurance)?

The administrative fee to complete forms not covered by insurance is $25.00.

Can I pay my bill online?

You can pay your bill on line via MyChart.

What if I can't afford the necessary co-pay?

If you are not able to pay the copay at the time of service, you will receive a statement in the mail.

Are you able to provide the cost of the appointment prior to my scheduled service?

The cost of your appointment will be based on your insurance benefits. If there is no insurance to bill, we will provide an estimated charge.

Do you offer payment plans or CareCredit?

We are able to set up a payment plan if needed. We do not accept CareCredit.

What if I can't afford to pay my bill?

If you are unable to pay your bill, you should contact our eligibility and enrollment services for assistance with the selected programs we provide.

What if my coverage changes?

Hours will vary by site. Some locations have evening and weekend hours. Please check with your doctor for more details.

What if I need to talk to my doctor after-hours or on holidays?

Call the physician office and you will be connected to a nurse answering service.

Is there a shuttle service from the hospital I can use?

There is no shuttle service.

Are your facilities wheelchair accessible?

All facilities are wheelchair accessible.
