How to Get Care at this Location
Your primary care provider will need to fax a referral to our clinic before you can make an appointment with a doctor at this location. Talk to your doctor or call our office if you have questions or need assistance.
Finding the Clinic
Enter the Professional Building II through the revolving front doors. Once inside, you’ll see three elevators directly in front of the main entrance. Take an elevator to the fifth floor. Exit the elevator and go left toward the clear doors at the end of the hallway to suite 502. For your convenience, we have a restroom inside of our clinic. This suite also houses neurology.
Public Transportation
If you take public transportation, get off at the Main and Morphy bus exit. Walk towards the two tall buildings in front of the bus stop named the JPS Professional Office Complex. Enter the Professional Building I on the left.

Visitor Policy
For everyone’s safety, clinic patients can only bring one visitor at this time. Anyone who comes to this location will be screened at the door for COVID-19 symptoms or recent exposure.
Please arrive at least 20 minutes before your appointment. Once you arrive, check in at the front desk.
