How to Make an Appointment
You will need your primary care provider or doctor to fax a referral to the clinic before you can make an appointment with a doctor at this location.
Finding the Clinic
Enter the Professional Building I through the sliding front doors. Take the main elevators to the fifth floor. Exit the elevators to the left. Walk towards Suite 501 with the clear doors at the end of the hallway.
Public Transportation
When arriving by public transportation, you should get off at the Main and Morphy bus exit. Walk towards the two tall buildings in front of the bus stop named the JPS Professional Office Complex. Enter the Professional Building I on the left.

For everyone’s safety, clinic patients can only bring one visitor at this time. Anyone who comes to this location will be screened at the door for COVID-19 symptoms or recent exposure.
Please arrive at least 20 minutes before your scheduled appointment. Check-in at the front desk.
