You’ll need a primary care provider’s or doctor’s referral to schedule an appointment with a doctor at this location. Talk to your doctor or call our office if you have questions or need assistance.
Finding the Clinic
Enter the Professional Building I through the sliding front doors. Go to your right and through the door to check-in at the clinic.
Public Transportation
When arriving by public transportation, you should get off at the Main and Morphy bus exit. Walk towards the two tall buildings in front of the bus stop named the JPS Professional Office Complex. Enter the Professional Building I on the left.

Visitor Policy
For everyone’s safety, clinic patients can only bring one visitor at this time. Anyone who comes to this location will be screened at the door for COVID-19 symptoms or recent exposure.
Please arrive at least 20 minutes before your appointment time.
